    I’m creating some service tickets. Do I need to copy some or any of the legal verbiage from the Monitoring Contract, onto the back of these forms as well?

    I can't endorse that.  There are a number of reasons.  

    Service Tickets are not a substitute for a Service Contract, which is part of the All in One contracts.  In order to insulate your company from liability you must have a contract in place that covers your services, all of them, including the sale and installation of the equipment,  before the work is performed.  One problem with a Service Ticket is that it is typically signed after the work is completed, and often by someone who may be home or available at a business, but not someone authorized to bind the subscriber - owner to a provision exculpating you from liability, including liability for your negligence.

    Another problem is that without seeing how you have designed your Service Ticket I can't evaluate if you would be properly protected even if you did get it properly signed.  We do offer a Service Ticket.  It's not on the order form at www.alarmcontracts.com but you can get it by calling our Contract Administrator Eileen Wagda at 516 747 6700 x 312.  While this Service Ticket can be used as a stand alone, it is recommended to be used together with a Service Contract.  You can use a separate Service Contract but a far better investment will be the All in One contract forms which cover all aspects of sale, installation and services.      There are now 4 All in One standard contract forms - which I think covers everything you do as an alarm / security / fire company.  They are the
