Ken,
    ACH and automatic credit card payments are extremely valuable attrition fighting tools. 
    Monthly - not quarterly, not bi-annual, not annual autopay billing with invoices sent via email keep the provider’s name front and center.  A quick message explaining features and new services creates good will and generates sales leads. 
    A rejected payment is an instant alert to a customer service issue, a death, a relocation or a financial problem. Getting that alert with 30 days to fix the problem (always bill in advance) is key in keeping attrition at bay. 
    For those who complain about 3% credit card fees take a moment and calculate the cost of accounts receivable personnel.  Simply put- autopay eliminates headcount.

    I agree that auto  pay is a much more efficient way to assure payment.  It does require personnel to process the monthly payment, but so would opening mail and depositing a check.  Usually there is no charge for ACH.  Many merchants require the customer  to  pay the surcharge on the credit card, usually 3%, and it likely deters only a  few, especially when the charge is only a few bucks, as it typically is for RMR payments.  
    You are right that auto pay is not only a great item for reducing attrition and non-payment, it also affords quick notice when something is awry, as you point out, like dissatisfaction, death or financial changes.  
    I also prefer monthly to other periodic payment schedule.  It's more timely and smaller numbers so less likely to be objectionable.
    The K&K Agreements come  with the ACH / CC form.
