Ken,
    Is it legal for a customer to just put their first name on a contract or does it have to have a last night? I know that may be a dumb question but figured I would ask.

    I have to start this way.  Are you in the part of the country where a belt buckle can be used for identification?  A belt buckle with only the first name, by the way?
    I don't think so.  Of course you can't just have a first name on a contract, and guess what, I don't want you to have only a last name on the contract either.  You need the person's full name; I'd prefer you get copy of driver's license or at least a library card.
    It essential that you get the proper name of the person signing.  It's essential that you get the full name of a corporate entity also, as well as the name of the person signing and that person's position with the company that permits him to sign the contract on behalf of the entity.
    I know you're thinking that you don't have to worry about the customer and you're not that concerned with getting paid.  Well how about that customer suffering a significant loss and the customer or its insurance company suing you for more than your insurance and more than your company is worth.  
    If you think you're going to get away by just showing your belt buckle when you get to court, re-consider.
