    Attached is the one I use.  Also if you use Pro Bill there is one in their document section.  Also, the 2 central monitoring centers I use will also send one on your request.  The only thing that I get asked for and is not on that form is the panel type and central's address.  I usually put that on the cover sheet when faxed out.

    I’m responding the Certificate of Insurance below, hope this helps. The monitoring company provides us the form attached which I don’t know if it’s a good idea to share it with other dealers or monitoring companies (We are only the dealer of Emergency 24).

re  Eric Williamson's question for alarm certificates.
    You get them through your alarm monitoring company. They all have them. Just go to their website and download it. You will find the companies name and logo on top and details with their address, phone numbers etc. A checklist is on the bottom for you to check off what the customer has. I have 3 different monitoring companies, due to buying 2 more alarm companies, and they all have them.
